The Community Aging in Place - Advancing Better Living for Elders program helps residents of Clay and Platte County who are 60 years of age and older who are having difficulties with daily activities such as bathing, dressing, grooming, eating, etc. View our services.

NHCA serves both Clay and Platte Counties. The Care Coordinator assists eligble residents get access to healthy food, navigate prescription costs, help with utility bills, and more.

Northland Shepherd Center assists Clay and Platte County adults age 60 years and older. Services provides includes transporation, meals, minor home repair, shopping assistants and more.

MO HealthNet’s Non-Emergency Medical Transportation program provides free rides for eligible MO HealthNet participants. For additional information and to find out if a participant is eligible, call or check out their website.

You can search for skilled nursing facilities on The Center for Medicare and Medicaid Services (CMS) by star rating. Go to Medicare.gov and search “Nursing Home Compare” to find the most current overall star rating for each facility. The star rating scale is 0-5, with 0 stars for those with the lowest performance, up to 5 stars for those with the highest performance. The ratings are based on performance measures in 3 categories:
