Community Health Needs
Creating a healthy Northland requires collaboration with local individuals and organizations. This Community Health Needs Assessment (CHNA) brought together health leaders, providers and residents to research and prioritize our community health needs. It serves as the foundation for North Kansas City Hospital and Meritas Health’s community health improvement efforts for the next three years with a special focus on health equity as it relates to transportation for healthcare services, mental and behavioral health services, access to healthy foods and cardiac health outcomes.

North Kansas City Hospital and Meritas Health became a founding member of the Northland Health Alliance (NHA) in 2014. The NHA brought healthcare stakeholders from across the community together around a shared goal of improving the health status and health outcomes of all Clay and Platte County residents. Read more about the collaboration and its members on pages 4-5 of the CHNA document.
FY2022-FY2025 Community Health Improvement Plan
North Kansas City Hospital completed an implementation plan in partnership with the Northland Health Alliance that was adopted by the hospital Board of Trustees in fiscal year 2022. The implementation plan connects hospital, community partners, clinicians and other resources with identified health needs. North Kansas City Hospital aligns health priorities with the areas of greatest identified need and considers where the hospital’s resources will generate the greatest impact.
