Appointments are recommended and are available Monday-Friday, 8 a.m.-5 p.m. Walk-ins are welcome at NKCH Campus and Independence Clinics Monday-Friday, 8 a.m.-4 p.m.

- Authorized employee or employer representative require a valid photo ID and completed paperwork upon arrival.
- Authorized employee and employer representative only are allowed in the clinic.
- Employer representative may attend injury visits with the employee's permission.
- Services after 4 p.m. require an appointment.
DOT Physicals
Please bring the following to your DOT exam:
- Driver's license (current)
- Glasses or contacts if needed
- List of your medications with dosages and frequency. Also be sure to take your prescribed blood pressure and diabetes medications at least 2 hours prior to your exam. This will help us to know how you are doing on your medications from your blood pressure and diabetes standpoint.
- Notes from your doctors if you know one is needed or think you think one maybe needed; example if history of cardiac (heart) problems an annual note from the cardiologist is recommended.
- Any special reports - such as an every other year stress test with an ejection fraction if you have a history of past Ml, stents, or known heart problems.
- Most current sleep study report and/or CPAP compliance report if you have obstructive sleep apnea and using a CPAP machine.
- Copies of any special labs - such as recent Hemoglobin AlC if diabetic; INRs if on Coumadin; kidney tests if history of kidney problems; or any other special tests that you have to have monitored for a health problem.
- Please bring any other information you think might be needed to complete your exam the day of the evaluation
- We also recommend avoiding caffeinated beverages, energy drinks and smoking prior to the exam
