Acute Rehab Patient Information
The following information gives you some ideas on how to prepare for and what to expect during your stay on our Acute Rehab Unit.
Consider bringing the following personal items from home:
- Assistive devices (walker, cane, braces or orthotics), if you already own them; please label with your name
- Comfortable, supportive shoes
- Eyeglasses/contacts, dentures, hearing aids
- Items for showering, if you prefer to use your own shampoo, conditioner and soap
- Loose-fitting, comfortable clothing (shirts/blouses, pants, shorts, underwear, nightwear, socks)
- Personal care items such as your toothbrush, comb, brush, electric razor and makeup
- Sweater or jacket
- Swimsuit, if participating in aquatic therapy
Meals are served in the Acute Rehab dining room at 7 a.m. (Breakfast), noon (Lunch) and 5:15 p.m. (Dinner), unless your nurse specifies differently. As part of the rehab program, patients are highly encouraged to go to the dining room for meals unless your care team suggests otherwise for medical reasons. Families are invited to eat with their loved ones.
Family conferences give patients and their families the opportunity to talk with the care team about goals, progress and discharge plans. To set up a conference, contact the unit social worker.
A therapeutic pass or home evaluation may be necessary before discharge. The assessment lets patients practice their new skills and gives us the opportunity to address additional discharge needs.
We encourage family and caregiver involvement throughout the rehabilitation program and discharge planning process.
Medicare pays for the cost of inpatient rehabilitation, provided the patient meets admission criteria and continued-stay criteria. Normal co-pays apply.
We will verify private insurance, Medicaid and managed-care benefits before admission and obtain pre-authorization. Authorization is not a guarantee of payment. Throughout a patient's stay, we will work to obtain continued authorization.
Patients must be at least 18 years old for admission.
