Heart Failure Readmission Rates Lower Than National AverageHeart failure (HF) affects nearly 5.7 million Americans, and it is the leading cause of hospitalization in people 65 and older. A heart failure diagnosis doesn’t mean your heart has stopped working, but it isn’t working as well as it should.

HF is also the primary reason patients return to the hospital following discharge. But North Kansas City Hospital’s Home Health telemonitoring program can help keep you healthy at home so you can maintain your independence and avoid yet another hospital stay.

We’ve made the program as easy as possible. You receive a scale, blood pressure cuff, and depending on your condition, a pulse oximeter to record your weight, blood pressure heart rate and oxygen levels. Each day, you check your vitals and submit your data over a secure telephone line (land or cell) to our monitoring software.

On the other end, our Home Health nurses review and evaluate your numbers and follow up, if needed. A nurse might check in with a phone call just to make sure you are OK or schedule a visit for a more thorough assessment, especially if something isn’t quite right with your numbers.

With in-home monitoring, our nurses can watch for early warning signs, such as weight gain, and catch potential problems sooner. And, it makes it easier and more convenient for you to take an active role in your health so you can remain at home and in control.

Our Home Health nurses work closely with our Heart Failure Clinic, the first of its kind in the Northland, to help you better understand the disease and what you can do to stay healthy at home.
