To request copies of your MH patient record, please contact your Meritas Health clinic directly. For your convenience, you can complete the MH release form prior to visiting your associated clinic.

To request copies of your North Kansas City Hospital patient record, visit the Medical Records office, located on the 1st Floor of the Pavilion, to sign a release. For your convenience, you can print and complete the release form prior to your visit.
You will not be charged for copies of medical records requested for personal use. If you are physically unable to visit the Medical Records office, please contact us at the below number.
