You’ll find primary care services for the entire family, with extended office hours, including commercial driver health exams—provided by a physician certified as a medical examiner by the U.S. Department of Transportation (DOT).

Your Appointment
Let’s make the most of your visit. Before your first appointment, please have your referring physician forward your medical records. Try to arrive 30 minutes before your first appointment, so we can verify your information. For future visits, arrive 15 minutes early.
What to Bring to Your Appointment
To each visit, please bring:
- A current, complete list of your medications
- Any applicable copay or co-insurance amounts
- Your insurance card, driver's license or id

Prescription Refills
There are a few ways you can request a prescription refill:
- At your appointment - Tell your doctor if you’ll need a refill before your next visit.
- From the patient portal - Log in to myhealth patient portal. Under My Medications, select Renew to request a refill.
- Through your pharmacy or mail-order service - Call your pharmacy or mail-order service for refills, and they will contact us for approval.
For any refills, be sure to request them several days before you run out of medication. We respond to refill requests during business hours, so it’s helpful to plan ahead.
After-Hours Calls & Emergencies
For urgent medical care after business hours, call 816.776.2201 to reach our on-call doctor. They can help with health questions and concerns. Note that we do not call in medication refills on evenings and weekends.

Patient Registration & Forms
To reduce the time spent at our office, print and fill out any necessary forms in advance. Bring them with you to your appointment.

To make the most of your appointment with a family doctor, cardiologist, or ENT doctor:
- Arrive 30 minutes before your first visit and 15 minutes before all subsequent appointments so we can collect, verify or update your information.
- Bring your insurance card and money for copays or co-insurance. (See what insurance providers we accept)
- Bring a current, complete list of your medications.
To request a refill between appointments, call your pharmacy or mail-order service. Ask your pharmacy about electronic prescription refills (E-Prescription). Submit your refill request several days before you run out of medication; we respond to refill requests only during our business hours.
To save time during your visit to our family practice office, print and fill out any necessary patient forms at home and bring them to your appointment.
