Your Psychiatrist Appointment

When you walk through our doors, you can expect a warm welcome from our staff, who is here to listen to your concerns. Help your appointment go as smoothly as possible by reviewing the following details.

Before your appointment, arrange to have your referring physician forward your medical records. Please arrive 30 minutes before your first appointment, so we can verify your information. For future visits, arrive 15 minutes early.

What to Bring to Your Appointment

To each visit, please bring:

• A current, complete list of your medications
• Any applicable copay or co-insurance amounts
• Your insurance card

Prescription Refills

There are a few ways you can request a prescription refill:

• At your appointment - Tell your doctor if you’ll need a refill before your next visit.
• From the patient portal - Log in to myhealth patient portal. Under My Medications, select Renew to request a refill.
• Through your pharmacy or mail-order service - Call your pharmacy or mail-order service for refills, and they will contact us for approval.

For any refills, be sure to request them several days before you run out of medication. We respond to refill requests during business hours, so it’s helpful to plan ahead.

After-Hours Calls & Emergencies

Use NKCH Emergency Care or go to your nearest emergency room if you need immediate care.

Call 911 in a medical emergency.

Patient Registration & Forms

To reduce the time spent at our office, print and fill out any necessary forms in advance. Bring them with you to your appointment.
