Apple Health


Apple HealthIf you have an iPhone with iOS 15, the Apple Health app, and an active myhealth patient portal account, you can connect your NKCH/Meritas Health electronic health record with the app to make sharing health data with your providers easier and more convenient. You still control what information you share and who can see it. Your weight, blood pressure numbers, glucose monitoring results and physicial activity level are just a few examples of the information you can share with your healthcare providers. 

Set Up

Apple Health share with providerOpen the Apple Health app and follow these steps.

  1. Tap "Sharing" at the bottom of the screen.
  2. Do one of the following:
    1. Set up sharing for the first time: Tap "Share with your doctor."
    2. Share with an additional provider: Tap "Share with another doctor."
  3. Tap "Next," then select one of the suggested providers, or use the Search function to find your provider. 
  4. If "Connect to Account" appears, tap it, enter the user name and password for the myhealth patient portal account, then follow the instructions.


    In addition to sharing your health data, when you connect a myhealth patient portal account to the Apple Health app, the health records for that account to download to the app.
  5. Choose the information you want to share with your healthcare provider. Scroll down to see all options. Tap "Next" to see the next screen.
  6. Tap "Share," then tap "Done."

Apple Wallet and Your Vaccination Record

Add a RecordWith your iPhone, you can store medical records of verifiable immunizations and test results in the Apple Health app. The patient portal, myhealth, supports the SMART Health Cards and allows you to download your verifiable COVID-19 vaccination or test result records by scanning the QR code.

  1. Log in to myhealth.
  2. Click "My Health Record," then "COVID Center."
  3. Click "View Vaccine Verification." 
  4. Scan the QR card or take a screenshot.

To request a connection to a different app other than Apple Health, please complete and submit the form below.
